NO. 37' ; s ATTACHMENT -A Units 8.& 9 (Management) 1. The expressed provisions hereof represent all the changes to be made in the terms and conditions of employment of the employees in.Vanagement. 2. City of Petaluma Group Dental, Insurance Program: The City shall provide for a.grou,p dental insurance coverage program for City employees in Management and 'their dependents. The:City shall pay during 1979-80, $21.85 per employee, per month, towards the City group dental insurance coverage program: 3. City of Petaluma Group 1ledi`ca'I-Hospital Insurance Program: The City shall provide for a group medical -hospital insurance coverage program for City employees in Management. The City shall pay during 1979-80, the following insurance premium amounts towards the City group medical -hospital insurance coverage program: (A.) Insurance premium amount„of $45.18 per month, per'employee:, towards employee coverage; and (B) For, emp'aoyees,who maintain dependents) c'overage,,, either: (a) Insur,an,ce prem;idm;' amount, of $24.97 per month, per emp.l:ogee, towards -,qpe (1 ),..dependent coverage for employees who ma i nt-Ai n o 6 (`1) dependent coverage°or (b) Insurance prem umm amount of $34.81 per month.,. per, ,employee, towards two ( 2) or more”' dependents- coverage for employees who maintain two (2) or more, dependents coverage. 4. Authorized, Ci:-ty observed, ten (.10') paid fixed -date holidays shall be as establ i;s.hed f:or the; Ci ty'' s fi seal; year as determi ned by 'Ci ty' Counci l Resol uti on. During the fiscal year 1979-80, the City will authorize one (1) "floating Holiday" per employee, which may -.be taken by the employee at a time selected by the employee, subject to operational requirements and approval as determined by the City. 5. The City.shall set the annual Min imum-Maximum salary amounts for each of the classifications included in''Management with the City Manager making any necessary adjustments as appropriva'te,,per einployee in those classifications, during the fiscal year, (except Council appointed) as set forth i'n Appendix "A", attached 'hereto and made a part hereof: 6.
