The expressed provisions hereof repre,sen,t all the changes to be made in the terms and conditions of employment of the employees.in Unit 1. 2. SALARY The City shall adjust the existing sal•ar,y ranges for each of the classifications included in Unit 1 by five percent (5%) effective July.1, 1976. 3. City of Petaluma Group Medical -Hospital Insurance - Program: The City shall provide for a medical -hospital insurance program. The City -agrees to pay the increased cost of medical -hospital insurance premiums. for the employee Eor 1976 -77, -which amounts to a total premium payment of $43.03 per month per employee. The City agrees to pay during 1976-77 the amount of $17_73 per month per employee -towards the cost of dependent (s) coverage for medical and hospital insurance premiums for all employees who maintain dependent (''s) coverage. 4. City of Petaluma Group Dental Insurance Program: The de for an employee group dental insurance program for shall provide during 1976-77 with the City agreeing to pay the amount of $6.34 per month per employee toward the cost of the program's insurance employee premium at the schedule of benefi-ts provided by the City program. 5. Accrued 'Sick Leave Usage: Leaves of ab:s'ence up to four working days with pay per calendar year may be granted to an employee in the event of serious illness or injury in the employee's immediate family and will be charged against sick leave. The iramed'iate family shall -consist of the spouse, children*, parents, brothers, sisters, or ether individuals whose relationship to the employee is that of a dependent or near dependent. In each case, the appointing power shall grant such sick leave only t4hen in his opinion, the relationship of the sick or di.sablbd person to the employee warrants: such u"se 'of sick leave. 6 Wox-ke.rs I Compensation: Benefits shall be payable in situations where employee absence is due to industrial injury as provided in California State Workers' Compensation Law. The amount of disability payments paid to the injured employee shall be deducted from salary payable to the employee while on sick leave. During the first three (3) calendar days of absence for industrial disability, employee's will be compensated at their 'full rate of pay without deduction from sick leave unless they are hospitalized. Absences resulting in immediate hospitalization or absences continuing beyond the third day'sball be charged against the employee at the rate of one-half (1/2) day for eachday of absence credited or compensated by workers' compensation insurance, provided -the City receives all compensation paid by workers' compensation insurance. The inj'ur'ed employee may choose to receive workers' compensation payments only, without City payment for salary at no loss of sick leave., Sick leave for industrial injury shall not -be- allowed for- a disability resu,l-t n'.g- fr--o-m-sickn-ess, self- inflicted injury or willful -misconduct. 7.Authorized, City observed, paid Holidays :1976-77: In fiscal year 1976=77, the City will designate ten (10) paid fixed -date holidays per employee as, listed below. .Additionally, during the fiscal year 1976-777 the City will -authorize .one Cl) paid floating holiday per employee, which may be taken. by the employee at a time selected by the employee, subject to operational requirements as determined by the Department Head.
